Standardized Authorization Form
All medical marijuana authorizations must be written on the Department of Health form dated June 2015 (or later). All previous medical marijuana forms and cards will not be accepted at the licensed and medically endorsed marijuana stores.
A valid authorization form allows medical marijuana patients included in the medical marijuana authorization database. Joining and receiving a medical marijuana recognition card is optional for medical marijuana patients who are over the age of 18. There are, however, some helpful benefits to consider.
Medical Cannabis Forms
For starters, recognition cardholders are able to purchase from medically endorsed retails stores without paying sales tax. You can buy up to three times the current limits at licensed retail stores and have access to high-THC options. You’re able to possess six plants and eight ounces of marijuana and get authorized for more (up to 15 plants and 16 oz of usable marijuana produced from plants) by a healthcare practitioner. Along with arrest protection, you can also participate in a medical marijuana cooperative.
American Harvest helps you through the process. Simply bring your authorization form and an approved form of identification to our fully licensed and medically endorsed retail shop in greater Leavenworth and surrounding areas. We have a certified medical marijuana consultant on staff who will enter your info into the database. We’ll need to take your picture, and then we can create a new medical marijuana recognition card.
